Infant/Child/Special Baptism Registration Form

Dear Parent(s)/ Guardian/ Sponsor,

Thank you for taking this important step in presenting your child to be baptised and affirming the commitment to nurture him/her in the Christian faith!

Before you begin with the registration, please prepare the following document for upload.

- A copy of your child’s Birth Certificate or Citizenship Certificate (for Singaporean born overseas). PDF format is preferred.
