Infant/Child/Special Baptism Registration Form

Dear Parent(s)/Guardian/Sponsor,

Thank you for taking this important step in presenting your child to be baptised and affirming the commitment to nurture him/her in the Christian faith!

Before you begin with the registration, you may want to prepare the following documents for uploading onto our system:

  1. A copy of your child’s Birth Certificate.
  2. A copy of either parent’s Wesley Membership Certificate
  3. If you are not a member but you are currently attending Baptism Membership Class (BMC), you have to email to the BMC Team to obtain verification of your BMC participation. Upon obtaining the email, you may upload the email in place of the Wesley Membership Certificate.
